There is a residual functional capacity form for these disorders which your attorney will submit to your treating doctor for completion. I think it is helpful if your doctor has your diary of when your attacks occur, what causes you to have an attack, how severe the attack is, how long the attack lasts, how it wipes you out and how you feel after an attack. If you have constant problems, outline the problems you have functioning, problems with positioning and acute attacks.
